We are currently accepting application forms for the 2024 summer season. Enrollment is confirmed with receipt of an acceptance letter via first class mail. An interview is required for any child new to our program.

This health history is to the best of my knowledge correct and the person herein described has permission to engage in all prescribed camp activities except as noted. In the event I cannot be reached in an emergency, I hereby give permission to the medical personnel selected by the Camp Director to order x-rays, routine tests, treatment and necessary transportation for my child to the physician selected by the Director to secure and administer treatment, including hospitalization for my child as named above. The completed form may be photocopied for trips out of camp.

Early Bird Savings:
Eligible for registering before March 1, 2024 for the full season.
- I am registering before March 1, 2024 for the full camp season. Please apply the $100 Early Bird Savings.
Additional Information
- A minimum of 5 children are required to provide bus transportation.
- There is a 50% Bus Transportation Sibling Discount. Please contact the camp office to redeem it.
- Additional weeks and options may be added at any time through the camp office, space permitting.
- $150 is credited for recommending new campers, upon their enrollment. Referring family must be listed on camper application upon submission.

Terms of Agreement
I hereby agree that:*
- ● I accept responsibility for payment of all camp fees.
- ● Payment in full, including transportation, is due before the start of camp. Any new or additional registration received after the start of camp must be accompanied by payment in full.
- ● A $300 deposit shall accompany this application (deposit will be deducted from the camp fee).
- ● No refunds, discounts or credit will be given for days not attended due to illness or private vacation.
- ● Berkowitz Gan Israel Day Camp reserves the right to refuse admission to any child at any time if it should deem necessary to do so.
- ● Berkowitz Gan Israel Day Camp reserves the right to terminate enrollment of any camper who does not demonstrate reasonable age appropriate standards of behavior (as determined by the camp).
- ● I give my child permission to participate in all camp activities both on and off site, unless otherwise specified.
- ● I understand and give my child (entering Nursery and up) permission to swim every week at Manorhaven Pool in Port Washington or other pool in event of unavailability.
- ● Complete Medical Forms must be in the possession of BGI two weeks before the first day of camp. By order of the Health Department, no child will be allowed in camp without completed medical forms on file.
- ● Camp staff has the authority to sign, on behalf of parent or guardian, any necessary waivers required to participate in scheduled trips and activities.
- ● In case of medical emergency, camp staff has permission to treat camper as deemed appropriate.
- ● Permission is granted for photographs and videos taken to be used for promotional purposes and in print publications, on camp website, and social media.
